چکیده

Purpose:BackgroundPrior studies indicate significant physiologic differences between obese and non-obese heart failure (HF) patients, but none have evaluated in hemodynamic patterns these patient populations during treatment for acute decompensated HF (ADHF). ObjectivesIn this study, we assessed trends patients ADHF.MethodsObese (BMI >30, n=63) <25, n=69) with ADHF the Evaluation Study of Congestive Heart Failure Pulmonary Artery Catheterization Effectiveness (ESCAPE) study who had pulmonary artery catheterization data available through duration were evaluated. Hemodynamics analyzed at baseline optimal day. Changes BNP, weight, creatinine, BUN, 6MWT, orthopnea dyspnea scores assessed.ResultsDespite similar hemodynamics, significantly less absolute relative arterial wedge pressure (PAWP) reduction (-16 ± 28 versus -32 29%, p = 0.04) treatment. Obese also higher PAWP (22.1 + 8 14.9 7.4 mmHg, p=0.001) PA pressures optimization compared patients. improvements six minute walk test distance, scores, changes creatinine BUN.ConclusionObese treated display invasively measured left filling pressures, despite symptoms, weight loss, noninvasive surrogates congestion (Figure). Our findings suggest a degree decoupling congestive symptoms :BackgroundPrior ADHF. assessed. Despite BUN.

CONGESTIVE HEART FAILURE IN CHILDREN: A SURVEY OF 114 PATIENTS

During a period of four years between June, IS, 1988 to June, IS, 1992 one hundred and fourteen patients with congestive heart failure (CHF) were admitted to the pediatric department of Taleghani General Hospital. During the above period, 192 patients with heart disease were hospitalized at this department and CHF was the cause of admission in 59.4% of them. Congenital heart disease (CHO) w...

Pulsatile hemodynamics in congestive heart failure.

Pulse pressure, an indirect measure of vascular stiffness and pulsatile load, predicts clinical events in congestive heart failure (CHF), suggesting that abnormal pulsatile load may contribute to CHF. This study was designed to assess more direct measures of central pulsatile load in CHF. Noninvasive hemodynamic evaluations were performed in 28 subjects with CHF and 40 controls using calibrated...

Exercise Hemodynamics and Metabolism in Severe Congestive Heart Failure

The acute hemodynamic and metabolic effects of amrinone during exercise were studied in eight patients with severe congestive heart failure. The patients exercised to exhaustion on an upright bicycle ergometer at a fixed work load of 25 W. During the control period of exercise, exhaustion occurred at 3.16 + 1.39 minutes. Stroke volume increased from 29.5 ± 5.8 to 38.7 + 9.5 ml (p < 0.01), arter...

Precipitating factors of congestive heart failure in admitted patients of Fatemieh Hospital in Semnan, Iran

Introduction: Congestive Heart Failure (CHF) is a pathophysiologic state in which an abnormality of cardiac function is responsible for the failure of the heart to pump blood at a commensurate rate with the requirements of the metabolizing tissues and/or allows it to do so only from an abnormally elevated diastolic volume. In this study, the prevalence of precipitating factors of CHF in Semna...

PATHOPHYSIOLOGY AND NATURAL HISTORY CONGESTIVE HEART FAILURE Relationship between central hemodynamics and regional blood flow in normal subjects and in patients with congestive heart failure

Central and regional (hepatic, renal, and limb) hemodynamic data are presented for a normal population (n = 16) and for a group of patients with congestive heart failure (n = 64). The patient population represented a wide spectrum of severity of congestive heart failure. Various relationships between central and regional hemodynamics were analyzed. The results indicate that in congestive heart ...
